What is the Purpose of the Competition?
The Issue:
Approximately 78 million adults in the United States have hypertension (high blood pressure) and about 17 million of those adults are unaware or undiagnosed. High blood pressure is a universal risk factor for heart disease and stroke, the leading causes of death and disability in the U.S. respectively. The American Heart Association wants people to understand how they can monitor and improve their heart health by using Heart360 and engaging in community-based health programs like Check. Change. Control.
The Program:
Check. Change. Control. Is a four month program that utilizes both online and offline components. Online, participants join Heart360, our FREE webbased health management tool. This tool allows an individual to track their numbers (blood pressure, glucose, cholesterol), as well as, track their risk reduction efforts (physical activity, weight). Offline, Check. Change. Control. offers participants education about hypertension, stroke, blood pressure management, nutrition and physical activity.
This program is an opportunity for organizations/groups to partner with the American Heart Association to provide FREE resources and education to the Madrinas and communities that they serve. All participating groups will have the knowledge that they are helping to save lives, build stronger communities and healthier families. The top three scoring Madrinas will even receive a monetary stipend!
Contest Rules
Eligibility:
Contest is open to Madrinas in the Tarrant County area that would like to create a culture of health in their community. Madrinas agree to enroll participants in the Check. Change. Control. Hypertension Management program.
Each Madrina will received a blood pressure cuff and will be asked to setup a station. One blood pressure cuff will be provided by the American Heart Association to every Madrina participating in the contest.
Contest Period:
Program runs 4 months
Winners announced on April 30th and awarded during the Vestido Rojo event

Prizes:
· First Place: $100 stipend and bragging rights!
· Second Place: $50 stipend
· Third Place: $25 stipend
* In the event of a tie, the stipend will be split equally between the winning parties.

Each Madrina must enroll each of their table guests into Check. Change. Control. For every additional person that you enroll, you will receive an additional point. The website to enroll is www.heart360.org/TCMADRINAS16

Madrinas will receive 1 point for each enrollee that records their blood pressure at least 2 times a month.
*Retained participants are defined as participants who enter their blood pressure readings www.heart360.org/TCMADRINAS16 for once a week for the duration of the program (16 weeks).
Each Madrina will be required to keep and send us monthly updates on your registration process. You will be asked to hand your final log with each of their enrolled participants. You will be asked to submit this when you register at Vestido Rojo. The log must include the participant’s first name and email address so we can verify.
